Fall Vaccines 2026-27: A Plain Guide to Flu, COVID and RSV

Fall shot season is here, and this year the advice is coming from more than one direction. A slim interim flu message from the CDC sits alongside a broader package covering flu, COVID-19 and RSV, issued by the nation's major physician groups. On the essentials, though, they mostly agree.

Who is issuing guidance

The CDC's vaccine advisory committee had not met in 2026, Healio reported, so four medical societies, including the American Academy of Pediatrics and the Infectious Diseases Society of America, published recommendations with the AMA. The AMA hosts a combined version, and a coalition of 16 Northeast public health agencies endorsed it on September 10.

Flu, COVID-19 and RSV at a glance

Flu: The physician groups recommend a shot for anyone aged 6 months or more unless there's a medical reason not to, and the CDC's interim guidance keeps yearly vaccination in place, calling September or October ideal for most people. Adults 65 and up are steered toward high-dose, recombinant or adjuvanted versions when available. Uptake among kids is lagging: just 49.4% of those between 6 months and 17 years old were vaccinated through May 16, 2026.

COVID-19: This is where messages split. The CDC tells parents of children under 18 to talk with their doctor, Scripps News reported, while the medical groups recommend vaccination for all adults, babies 6 to 23 months, and pregnant or breastfeeding people. For adults 65 and older and immunocompromised people, they advise a follow-up dose six months later.

RSV: For adults it is generally a single dose, recommended for everyone 75 and older and for people 50 to 74 at higher risk. Expecting parents can protect a newborn with a maternal vaccine at 32 to 36 weeks of pregnancy, or infants under 8 months can receive a long-acting antibody.

Timing

Dr. Amesh Adalja, an expert at the Johns Hopkins Center for Health Security, suggested mid-to-late October, before Halloween, as a good flu target, adding that a September shot beats skipping it entirely. If you have been waiting for perfectly matched guidance, the practical window is already open.
